29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation shall not be performed by the customer or user without manufacturers prior written approval. Capacity, operation, and maintenance instruction plates, tags, or decals shall be changed accordingly. On or about March 1, 2023, at the jobsite located at 990 Valley Road, Menasha, Wisconsin; an employee was operating a Hyster forklift (Model No.: H50UT, Serial No.: A3C1A07725V) with a Caldwell boom lift (Model No.: PB-60, Serial No.: 111878), which affected the capacity and safe operation of the forklift, without having received prior approval from the forklift manufacturer and the capacity plate was not changed accordingly.
