On or about March 14, 2023, the employer did not ensure the electrical energy isolating device (cord and plug) was operated in a manner to isolate the electrical power of a Kreuzmayr bottling line 1250, during service and maintenance operations.

On or about March 14, 2023, the employer did not ensure the operator of a Kreuzmayr Double Belt Press K2B1000, was protected by one or more methods of machine guarding from the hazards created by the point of operation and moving and rotating parts of the machine; including but not limited too; belt press. 2. On or about March 14, 2023, the employer did not ensure the operator of a Kreuzmayr Juice bottling line, was protected by one or more methods of machine guarding from the hazards created by the point of operation and moving and rotating parts of the machine; including but not limited too; point of operation.
