29 CFR 1910.303(b)(1)(ii): Electrical equipment was not free from recognized hazards, based on the mechanical strength and durability, including, for parts designed to enclose and protect other equipment, the adequacy of the protection thus provided: a. United States Postal Service - West Toledo Station: On or about March 14, 2023, the flexible power cord for the pendant control, used to raise/lower the dock lift, exposed employees to electrical hazards. The cord's sheathing was separated and/or pulled from parts it was designed to protect. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

29 CFR 1910.305(e)(1): Cabinets, cutout boxes, fittings, boxes, and panelboard enclosures in damp or wet locations were not installed to prevent moisture or water from entering and accumulating within the enclosures and metallic cabinets, cutout boxes, fittings, boxes, and panelboard enclosures in damp or wet locations were not mounted so that there was at least 6.35-mm (0.25-in) airspace between the enclosure and the wall or other supporting surface a. United States Postal Service - West Toledo Station: On or about March 14, 2023, the electrical junction box for the dock lift control pendant was not installed for damp locations. Employees that used the pendant control to operate the freight and man lift were exposed to electrical hazards in that the junction box for the control, located at the ceiling of the dock's overhang, was damaged and did not prevent moisture from entering the enclosure. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.

On or about February 23, 2023, an employee sustained an injury that resulted in lost time; injury and illness records did not include the job title, description of injury/illness, parts of body affected, classification of case, number of days away/restricted, and injury type.
