29 CFR 1910.22(a)(2): The employer did not ensure that the floor of each workroom is maintained in a clean and, to the extent feasible, in a dry condition. The employer did not ensure that when wet processes are used, drainage is be maintained and, to the extent feasible, dry standing places, such as false floors, platforms, and mats are provided. a. In the cabinet fabrication shop at 4 Aviator Way, Ormond Beach, Florida: on or about March 14, 2023, the employer exposed employees to possible combustible dust fires and secondary explosion hazards, in that wood saw dust had accumulated on the saw and cabinet making process equipment, surrounding floors, horizontal angle iron equipment and dust collection systems bracing , horizontal metal ducting and horizontal metal and wood framing trusses in excess of 1/32-inch.

29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years: a. On the property and in the cabinet shop area of the establishment at 4 Aviator Way, Ormond Beach, FL: On or about March 14, 2023 and at times prior thereto the employer exposed employees to struck-by hazards in that employees were allowed to operate a powered industrial truck without the employer ensuring that the operators had received the required evaluation at least once every three years as specified in the regulation.
