29 CFR  1910.134(f)(2):  The employer did not ensure that employees using  tight-fitting respirators were fit-tested prior to initial use, or whenever a different respirator facepiece (size, style, make or model) was used, and at least annually thereafter:  (a)Bullseye Custom Cabinetry, Inc. at 1015 South Hillside Drive, Sioux Falls, SD  57110:  On or about and at times prior to March 14, 2023, the employer did not ensure that employees were protected from respiratory illness in that fit-testing was not performed at least annually.  This condition exposed employees to health hazards due to inadequate respirator fit.

29 CFR  1910.134(g)(1)(i)(A): Respirators with tight-fitting facepieces were worn by employees who had facial hair that came between the sealing surface of the facepiece and the face or that interfered with valve function:  (a)Bullseye Custom Cabinetry, Inc. at 1015 South Hillside Drive, Sioux Falls, SD  57110:    On or about and at times prior to March 14, 2023, the employer did not ensure that employees were protected from respiratory illness in that the employer did not prohibit a tight-fitting facepiece to be worn by employees who had facial hair that came between the sealing surface of the facepiece and the face.  This exposed employees to health hazards associated with loose or ill fitting protective respiratory equipment.

This condition exposed employees to health hazards associated with respirator damage and/or contamination.
