On or about March 13, 2023, and at times prior thereto, employees were working in close proximity to a Pack West Auto 200 Torquer that had rotating parts including a belt and pulley, exposing employees to the hazard of being caught in the torquer machine.

29 CFR  1910.134(k)(6): The employer did not provide the basic advisory information on respirators, as presented in Appendix D of 29 CFR 1910.134, in written or oral format to employees who wear respirators when such use was not required by the employer.  On or about March 13, 2023, and at times prior thereto, the employer failed to provide voluntary respirator users who were working around spices that caused respiratory symptoms with a copy of Appendix D of the standard.
