29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use: On or about 3/3/2023 at 3221 Enterprise Way Miramar, Florida, three employees were exposed to a 34-foot fall hazard while being lifted on a wooden box with one side open, which was supported by a powered industrial truck. None had personal fall protection and the box was not secured to the forklift.

Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system: On or about 3/3/2023 at 3221 Enterprise Way Miramar, Florida, three employees were exposed to a 34-foot hazard while working on a low slope roof without a conventional fall protection method.
