29 CFR 1926.25(a): During the course of construction, alteration, or repairs, debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures Location: Throughout the jobsite in the worksite adjacent to 80 Braintree St., ALLSTON, MA 02134 On or about February 14, 2023 employees were exposed to fall hazards, where general housekeeping was an issue.

29 CFR 1926.28(a):Appropriate personal protective equipment was not worn by employee(s) in all operations where there was exposure to hazardous conditions location: the norther end of the jobsite, within the boundaries of the foundation walls On or about February 14, 2023 and during the operation of the mini-excavator on site, the operator was observed by the CSHOs not wearing his seatbelt demonstrating that the employer did not ensure that the employee was wearing his seatbelt while operating the excavator and demonstrating the failure to use appropriate personal protective equipment in all operations where there was exposure to hazardous conditions
