29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns.   At the jobsite, employees were not wearing hardhats while working below other employees that were roofing a residential house, exposing employees to struck-by hazards from falling objects.  Juvenal Salazar Moreno  was previously cited for a Serious violation of this occupational safety and health standard or its equivalent standard 1926.100(a)(1), which was contained in OSHA inspection number 1331041, Citation Number 1, Item number1, and was affirmed as a final order on March 29, 2019, with a final abatement date on August 18, 2018,  with respect to a workplace located at 7961 Ballater Drive Fort Worth, TX 76123.

29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.  At the jobsite, employees operating a Metabo nail gun to perform roofing activities without eye and face protection exposing him to the hazard of being struck-by flying objects.  Juvenal Salazar Moreno was previously cited for a Serious violation of this occupational safety and health standard or its equivalent standard 1926.102(a)(1), which was contained in OSHA inspection number 1331041, Citation Number 1, Item number 2, and was affirmed as a final order on March 29, 2019, with a final abatement date on August 18, 2018,  with respect to a workplace located at 7961 Ballater Drive Fort Worth, TX 76123.

29 CFR 1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest system.  At the jobsite, employees installing roofing material to a new residential house were not provided fall protection and exposed to an unprotected fall hazard at a height of over 6 feet to the ground below.  Juvenal Salazar Moreno was previously cited for a Serious violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1331041, Citation Number 1, Item number 3, and was affirmed as a final order on March 29, 2019, with a final abatement date on August 18, 2018,  with respect to a workplace located at 7961 Ballater Drive, Fort Worth, TX 76123.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  At the jobsite, employees performing roofing activities without fall protection were not provide a training program with hazard recognition and avoidance training where part of the job is to be exposed to fall hazards at a heights over 6 feet to the ground below.
