29 CFR  1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:   5219 Rogers Rd, San Antonio, TX, 78251: On or about March 15, 2023, and times prior thereto, employees were exposed to a struck-by hazard while using a pneumatic nail gun without wearing eye protection.

29 CFR  1926.1053(b)(1):Because of the portable ladder's length, the ladder side rails did not extend to at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access and the ladder was not secured at its top to a rigid:  5219 Rogers Rd, San Antonio, TX, 78251: On or about March 15, 2023, and times prior thereto, employees were exposed to a fall hazard while using a job-made ladder that did not extend 3 feet above the landing surface.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  5219 Rogers Rd, San Antonio, TX, 78251: On or about March 15, 2023, and times prior thereto, employees were exposed to fall hazards while conducting framing activities greater than 6 feet without utilizing fall protection.   Zapotitan LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(10), which was contained in OSHA inspection number 1605578, citation number 1, item number 2 and was affirmed as a final order on 12/19/2022, with respect to a workplace located at 12727 N. US HWY 281, San Antonio, TX 78216.

29 CFR  1926.503(a)(1):The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  5219 Rogers Rd, San Antonio, TX, 78251: On or about March 15, 2023, and times prior thereto, employees were exposed to a fall hazard while working at an elevation greater than 6 feet, without being trained on fall hazard recognition.  Zapotitan LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.503(a)(1), which was contained in OSHA inspection number 1620311, citation number 1, item number 2 and was affirmed as a final order on 12/19/2022, with respect to a workplace located at 8212 Talley Rd., San Antonio, TX 78253.
