29 CFR  1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.  (a) Worksite: On or about 03-16-2023, the employer did not establish a lockout/tagout program consisting of energy control procedures, employee training and periodic inspections.

29 CFR  1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years.  (a) Worksite: On or about 3/16/2023, the employer did not ensure that each forklift operator received a forklift skills evaluation once every three years.  Sandberg Enterprises, Inc. was previously cited for a violation of this occupational health and safety standard, 29 CFR 1910.178(l)(4)(iii), which is contained in OSHA inspection number 1302727, citation  number 2, item 2b and was affirmed on a final order on 4/10/2018, with respect to the workplace located at 806 Broncos Highway, Mapleville, RI  02839.
