29 CFR 1910.119(d)(3)(i)(B): The employer's piping and instrument diagrams were not accurate and did not represent equipment that was existing and was part of the process. a) For engine room number 2-3 at the Coca-Cola Auburndale, FL site, on or about 3/16/23, a ? inch Hansen HGV1 Long Neck Service/Gauge Valve located on piping associated with suction accumulator V-33 and downstream of an 8-inch isolation/stop valve, RL-1, was not depicted on the employer?s piping and instrumentation diagrams. b) For engine room number 2-3 at the Coca-Cola Auburndale, FL site, on or about 3/16/23, pressure gauge at a ? inch service/gauge valve on piping associated with suction accumulator V-33 and downstream of an 8-inch isolation/stop valve, RL-1, was not depicted on the employer?s piping and instrumentation diagrams. c) For engine room number 2-3 at the Coca-Cola Auburndale, FL site, on or about 3/16/23, valve RL-1/CH-31 was incorrectly field-labeled with respect to piping and instrumentation drawing number V-33A. d) For engine room number 2-3 at the Coca-Cola Auburndale, FL site, on or about 3/16/23, five valves at high pressure receiver V23 had illegible valve tags for valves depicted on piping and instrumentation drawing number V-23. e) For engine room number 2-3 at the Coca-Cola Auburndale, FL site, on or about 3/16/23, chiller CH-31 and isolation/stop valve RL-1/V-33 were located on the wrong side of the suction accumulator V-33 with respect to the level column on the piping and instrumentation drawing number V-33A.

29 CFR 1910.119(d)(3)(ii): The employer did not document that equipment complies with recognized and generally accepted good engineering practices. a) For engine room number 2-3 at the Coca-Cola Auburndale, FL site, on or about 03/15/2023, the employer exposed employees to inhalation and skin contact hazards associated with a catastrophic release of pressurized liquid anhydrous ammonia, in that, a pressure gauge and its 1/4 inch service/gauge valve on a 25,000 lb. approximate ammonia suction accumulator vessel V-33 was inoperable and not able to be read. Line breaking activities were commencing at/near the gauge location, and contractors involved were unable to get an accurate read of the pressure inside the pipe where line breaking was going to commence.

29 CFR 1910.119(j)(4)(i): The employer did not perform inspection and tests on process equipment. a) For engine room number 2-3 at the Coca-Cola Auburndale, FL site, on or about 3/16/2023, the employer failed to perform inspections and tests on isolation/stop valve RL-1/V-33. b) For engine room number 2-3 at the Coca-Cola Auburndale, FL site, on or about 3/16/2023, the employer failed to perform inspections and tests on a Hansen HGV1 Long Neck ?? gauge/service valve which was installed downstream of isolation/stop valve RL-1/V-33 on suction accumulator 33. A Gritty material had accumulated in the gauge/service valve. c) For engine room number 2-3 at the Coca-Cola Auburndale, FL site, on or about 3/16/2023, the employer failed to perform inspections and tests on a Hansen HGV1 Long Neck ?? gauge/service valve which was installed downstream of isolation/stop valve RL-1/V-33 on suction accumulator 33. The gauge was inoperable and unable to be read.
