29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders, as necessary, to train each employee in the procedures to be followed to minimize hazards related to ladders. Location: 8 Bradford Road, Hamilton, MA; On or about 03/16/2023, employees and employer representative (foreman) were witnessed utilizing a ladder which had a materials lift outfitted to it, causing the lower portion of the ladder to be blocked from safe access and egress. This was also the only ladder witnessed erected on site for employees to utilize for access and egress to the roof.
