29 CFR 1910.212(a)(1): One or more methods of machine guarding were not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. On or about February 28, 2023, in several locations on the site shops managed by Fabricated Steel Products, employees used several machines to perform machining operations without or inadequate guards : Machine #1: FMB Horizontal Band Saw Phoenix Model south side of bldg # 3 used in machine operations without an adequate guard attached. Machine #2: Coper Miter west side of Bldg #1 used in machine operations without an adequate guard attached.

29 CFR 1910.334(a)(2)(i): Portable cord and plug-connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket). On or about February 28, 2023, employees engaged in machining operations were exposed to electrical shock hazards due to the employer failing to visually inspect the flexible cord used to power a pedestal fan that had a damaged power cord. Damaged cords found in various areas: a. Bldg #1 West side of the building. b. Bldg #3 East side rear of the building.
