29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: On or about March 8, 2023, and at times prior to, front side of a residential dwelling under construction, an employee conducted framework installation without prior instruction in recognizing and avoiding unsafe working conditions. The employee was exposed to fall hazards when utilizing an inadequate scaffold, no use of hard hats while exposed to impact hazards, and accessing the scaffold from a ladder that did not extend above the landing.
