1926.503(c)  When the employer has reason to believe that any affected employee who has already been trained does not have the understanding and skill required by paragraph (a) of this section, the employer is to retrain each such employee such as inadequacies in an affected employee�s knowledge or use of fall protection systems or equipment indicate that the employee has not retained the requisite understanding of skill.  a.  On a 3:12 pitched roof of a temporary commercial work site, at 3530 CR 315 - Spring Park Elementary School. in Green Cove Springs, Florida 32043: On or about March 15, 2023, the employer exposed employees to fall hazards ranging from 14 feet to 24 feet 2-inches, in that fall protection was not utilized by all employees while performing roofing activities.

29 CFR  1904.41(a)(1): The employer had not electronically submitted information from their recording forms required under this part once and year and no later than the date listed in paragraph (c) of this section:  a.  A  & T Roofing and Exteriors, LLC, located at 11082 Englenook Drive, Jacksonville, Florida 32246; On or about March 15, 2023, the employer failed to report calendar year 2022, to electronically submit information from the OSHA 300 Form 300A or equivalent by March 2, 2023.  The establishment employed 28 employees and was classified in the North American Industrial Classification System as 238160 during calendar year 2022.
