On or about March 17, 2023, at a job-site located at 6315 Beth Ave SW, Navarre Ohio 44662; the employer did not ensure employees were adequately protected from rolling and or falling debris from a spoil pile that was located directly at the edge of the West side of the trench. An employee was working in the 6 foot deep trench connecting new yellow gas line directly below the loose material at the edge of the trench.  Miller Pipeline was previously cited for a violation of this occupational safety and health standard or its equivalent standard [1926.651(j)(2)] Specific Excavation Requirements, Spoil Pile), which was contained in OSHA inspection number 1632814, citation number 01, item number 01 and was affirmed as a final order on (1/31/2023), with respect to a workplace located at 201 Summer Valley/Kill Creek Rd, Gardner KS, 66030.
