29 CFR  1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure:   a) Job site, 7207 McKnight Road, Pittsburgh, PA: On or about March 16, 2023, during roofing activities, the employer did not ensure that employees used appropriate fall protection systems/devices where fall hazards of up to at least approximately 40 feet existed.

29 CFR  1926.454(c): When the employer had reason to believe that employees lacked the skill or understanding needed for safe work involving the erection, use or dismantling of scaffolds, the employer did not retrain each employee so that the requisite proficiency was regained:    a) Job site, 7207 McKnight Road, Pittsburgh, PA: On or about March 16, 2023, during roofing activities, the employer did not ensure that employees were provided with aerial lift re-training where unsafe work practices including, but not limited to, climbing over basket guardrails to get into the lift and failing to use fall protection while in the lift, were being practiced by employees.
