29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury. Location: Worksite On or about February 23, 2023 the employer failed to instruct each employee exactly which units had been disconnected and ready to be worked on that day, to ensure that all employees understood that machines to be demolished had their covers removed and were marked "DEAD". An employee attempted to demo a live unit with a sawzall and received an electrical shock.
