The employer representative responsible for scaffold erection at the site did not meet the qualifications of a competent person. This condition exposed employees to scaffold collapse and fall hazards.

Employees were walking and working atop the deck joists while laying plywood sheeting. This condition exposed employees to fall hazards greater than 6 feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: (a) PM Construction LLC at 14871 W. 82nd, Arvada CO 80007: On or before 3/17/23, the ladder used to access the upper work surface did not extend at least 3 feet above the deck. This exposed the employees to fall hazards while transitioning from the ladder to the deck at heights above 6 feet.
