29 CFR 1926.451(f)(7): Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration:  (a) PM Construction LLC at 14871 W. 82nd, Arvada CO. 80007: On or before 3/17/23, the employer did not ensure that scaffolding was erected, dismantled or altered under the direction of a competent person qualified in scaffold erection.  The employer representative responsible for scaffold erection at the site did not meet the qualifications of a competent person.  This condition exposed employees to scaffold collapse and fall hazards.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards:  (a) PM Construction LLC at 14871 W. 82nd, Arvada CO. 80007: On or before 3/17/23, the employer did not ensure that employees were adequately trained in scaffolding erection, dismantling, modifying or fall protection requirements. This condition exposed employees to fall hazards and scaffold collapse hazards.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  (a) PM Construction LLC at 14871 W. 82nd, Arvada CO. 80007: On or before 3/17/23, the employer did not protect employees from injury in that employees decking the patio were not using one or more methods of fall protection. Employees were walking and working atop the deck joists while laying plywood sheeting. This condition exposed employees to fall hazards greater than 6 feet.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   (a) PM Construction LLC at 14871 W. 82nd, Arvada CO 80007: On or before 3/17/23, the ladder used to access the upper work surface did not extend at least 3 feet above the deck. This exposed the employees to fall hazards while transitioning from the ladder to the deck at heights above 6 feet.
