Location:  3 Aston Court, Andover, MA 01810;  On or about 03/17/2023, subcontractor employees were exposed to fall hazards on the jobsite as follows: Instance a) Employees were permitted to work from job made scaffolding with no mean of fall protection.  Instance b) Employees were permitted to work in the direct vicinity of unguarded window and door openings which were greater than 6-feet above a lower level and no mean of fall protection was provided.
