29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: a) On March 15, 2023; the employer did not ensure a pipe layer installing a cluster valve in an excavation measuring approximately 8 - 9 feet in depth, with nearly vertical walls, had access to a ladder. The employee used the shallow trench with depths up to 5.2 feet to exit the excavation by jumping up on the wall and crawling out of the shallow trench to ground level. Note* Abatement certification AND documentation ARE required for this item.
