On or about March 21, 2023 at the jobsite located at 275 W Wisconsin Ave, Milwaukee, WI 53203, employee was observed using a 10 foot self-supporting (A-frame) ladder in the closed position to access the upper landing above the Kawa Sushi and Ramen vendor station, the ladder did not extend above the upper landing and was not secured to the landing exposing employee to a fall hazard.
