29 CFR  1926.451(g)(1)(vii):Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:  a) Jobsite: 661 Jefferson Blvd. Warwick, RI 02886: On or about 3/20/23 the employer failed to ensure that employees conducting work from an Alum-A-Pole scaffold, on the south side of the building, were protected from falls. The scaffold was erected without a mid rail and did not have a guard rail system installed on the ends of the work platform. The fall height was approximately 17 feet from the work platform to the cement/blacktop below.   b) Jobsite: 661 Jefferson Blvd. Warwick, RI 02886: On or about 3/20/23 the employer failed to ensure that employees conducting work from an Alum-A-Pole scaffold, on the east side of the building, were protected from falls. The scaffold was erected without a mid rail and did not have a guard rail system installed on the ends of the work platform. The fall height was approximately 17 feet from the work platform to the cement/blacktop below.

The fall height was greater than 17 feet and the roof slope was 9/12.
