On or about March 21, 2023, at the jobsite located at 1265 Woodview Dr. Waukesha, WI 53189 employees were installing siding on residential home from a pump jack scaffold without the use of fall protection, exposing employees to an approximately twelve (12) feet fall from the area or operation.
