Instance a.) On or about 03/08/2023, at the facility located in Enid Oklahoma on the roof of building #1, employees were exposed to tripping hazards by walking over gas piping to condensers and other equipment.  Instance b.) On or about 3/08/2023, at the facility located in Enid Oklahoma on the roof of building #1, employees were exposed to tripping hazards by walking over anhydrous ammonia pipelines to evaporative condensers EC-1 and EC-3.

29 CFR 1910.28(b)(1)(i)(A): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level was protected from falling by a standard guardrail system.   Instance a.) On or about 3/08/2023, at the facility located in Enid Oklahoma on the lower platform of the evaporative condenser 3 (EC-3) on the roof of building #1, the southside of the walking-working surface did not have a complete guardrail system installed.  This exposed the employees to a fall hazard greater than four feet.

Instance c.) On or about 6/21/2023, at the facility located in Enid Oklahoma on the platform of valve area in the machinery (engine) room of the ammonia refrigeration system, employees checking conditions of valves were exposed to a fall hazard more than four feet where the ladderway platform hole was not equipped with a self-closing gate or an offset.  Instance d.) On or about 6/21/2023, at the facility located in Enid Oklahoma on the platform in the hydraulic room, employees monitoring and/or checking conditions of vessels were exposed to a fall hazard more than four feet where the ladderway platform hole was not equipped with a self-closing gate or an offset.

29 CFR 1910.119(d)(3)(ii): The employer did not document that equipment complies with recognized and generally accepted good engineering practices (RAGAGEP).   Instance a.) On or about 3/08/2023, at the facility located in Enid Oklahoma, employees were exposed to ammonia chemical release hazards when the employer did not document that it complied with its chosen recognized and generally accepted good engineering practices, such as ANSI/IIAR-2 2019 (International Institute Ammonia Refrigeration) section 6.14.3.1 when the mechanical exhaust ventilation system was designed to produce less than the required temperature control ventilation rate.  Employees were exposed to accumulation of contaminants, heat and respiratory hazards.

This exposed employees to ammonia release hazards.
