Location: 25 Sixth Street, Chelsea, MA;  On or about 03/21/2023, the crane operator, who is listed as a competent person, utilized the Tadano GR800XL (serial number: 547134) crane without verifying the outriggers positions and had input into the LMI/AML that the jib was installed when there was no jib mounted to the crane or on-site.

Location: 25 Sixth Street, Chelsea, MA;  On or about 03/21/2023, crane operator utilized the Tadano GR800XL (serial number: 547134) crane against manufacturer procedures for safe operations in the following ways; - Not equipping the 17,400lb counterweight which was mandatory for operation per the manufacturer and industry standard practices. - Not following applicable load charts and configurations contained in the operations manual. - Bypassing the limiters (safety devices) such as the Outrigger Override, Jib Selector Key, and the LMI/AML Bypass.
