29 CFR  1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.  a) On or about March 7, 2023, the employer did not ensure that a rotating cutting blade was properly guarded on the Kalamazoo manufactured Metal Cutting Band Saw Model 8-C-W , located in the tool room, in that an unused portion of the front of the saw was unguarded, exposing employees to amputation hazards.  b) On or about March 7, 2023, the employer did not ensure that each employee operating the Clausing manufactured lathe, located in the tool room, was protected from the machines rotating parts.  As a result, employees were exposed to caught-on injuries from the rotating chuck while engaged in machining activities.  c) On or about March 7, 2023, the employer did not ensure that the stationary belt sander, located in the grinding room, had proper guarding on the unused portions of the sanding belts runs. As a result, employees were exposed to caught-in hazards associated with the nip-points of the machines rotating belt and pulley.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR  1910.215(b)(9): Exposure adjustment. Safety guards of the types described in Subparagraphs (3) and (4) of this paragraph, where the operator stands in front of the opening, shall be constructed so that the peripheral protecting member can be adjusted to the constantly decreasing diameter of the wheel. The maximum angular exposure above the horizontal plane of the wheel spindle as specified in paragraphs (b)(3) and (4) of this section shall never be exceeded, and the distance between the wheel periphery and the adjustable tongue or the end of the peripheral member at the top shall never exceed one-fourth inch.   a) On or about March 7, 2023, the employer did not ensure that the tongue guard on an abrasive wheel machine was adjusted to the constantly decreasing diameter of the wheel.  The Dewalt manufactured bench grinder, located in the center aisleway of the production area, exposed employees to struck-by hazards as the tongue guard was not adjusted to ? inch above the wheel surface.   Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR  1910.147(c)(4)(i): Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section.    a) On or about March 7, 2023, the employer failed to document and utilize energy control procedures for the Toshiba manufactured injection molding machine in the production area. As a result, maintenance employees were exposed to caught-between, struck-by, thermal burns, or electrical shock hazards associated with the equipment's unexpected energization or release of stored energy during service and maintenance activities.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
