29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii) A. On or about 3/21/23, at the site of 33-25 149th Street. Flushing, NY Employees removing siding on the fabricated frame scaffolds were exposed to the hazard of falling approximately 18 ft. to the ground. The working platforms were installed with only three scaffold planks.

Flushing, NY Employees working from tubular welded frame scaffolds were exposed to a fall hazard of approximately 18 ft. while removing siding to face of 2-story within the attic of the house.
