29 CFR  1903.19(d)(1): The employer did not submit to the Agency documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required.   a)  Auburn Community FCU, 7538 Oswego Road, Liverpool, NY  13088 - On or about 3/22/23:  The employer failed to provide abatement documentation for citation 1-3 identified in the inspection of that workplace.    Abatement documentation must be submitted for this item.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a)  Auburn Community FCU, 7538 Oswego Road, Liverpool, NY  13088 - On the roof.  On or about 3/22/23, workers were exposed to falls while installing roofing materials onto a 4:12 pitch roof with a ground to eave height of 13 feet without fall protection.

29 CFR  1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level:  a)  Auburn Community FCU, 7538 Oswego Road, Liverpool, NY  13088 - On the roof.  On or about 3/22/23, workers were installing roofing materials onto a 4:12 pitch roof with a ground to eave height of 13 feet. The workers were exposed to greater than 6-foot free-fall or striking the ground as the personal arrest system was not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level.

29 CFR  1926.503(a)(2)(ii): The employer did not assure that each employee, as necessary, had been trained by a competent person in the correct procedures for erecting, maintaining, disassembling and inspecting the fall protection systems to be used:   a)  Auburn Community FCU, 7538 Oswego Road, Liverpool, NY  13088 - On the roof.  On or about 3/22/23, workers were installing roofing materials onto a 4:12 pitch roof with a ground to eave height of 13 feet.  The workers were exposed to fall hazards. The employer did not ensure that each employee, as necessary, had been trained by a competent person in the correct procedures for erecting, maintaining, disassembling, and inspecting the fall protection systems to be used.   Abatement documentation must be submitted for this item.
