Gardner Denver dba LeROI Gas Compressor, located at 211 E. Russell Rd., Sidney, Ohio 45365:  On or about March 17, 2023, the employer did not ensure an employee operating a forklift was formally trained in the operation of power industrial vehicles. An evaluation of competency for the employee operating the forklift was not conducted and documented by the employer. The operating employee and others working around the forklift were potentially exposed to struck-by injuries due to operation by an unevaluated operator.
