Location: Facility  On or about February 27, 2023, the employer did not notify OSHA of a work related injury resulting in the amputation of an employees thumb.  OSHA was notified of the amputation by the Massachusetts department of Public Health of a workplace amputation.  The Home Depot notified OSHA of the amputation while onsite to conduct an inspection relating to the DPH notification.  The Home Depot Inc, was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1904.39(A)(2)), which was contained in OSHA inspection number 1440630, citation number 2, item number 1 and was affirmed as a final order on 4/13/2020, with respect to a workplace located at 1900 W state Road 426 Oviedo, FL 32765.
