1910.119(f)(4) The employer shall develop and implement safe work practices to provide for the control of hazards during operations such as lockout/tagout; confined space entry; opening process equipment or piping; and control over entrance into a facility by maintenance, contractor, laboratory, or other support personnel. These safe work practices shall apply to employees and contractor employees. a) Arkema-Sartomer West Chester, PAD A: On or about March 21, 2023, The employer failed to develop and implement a written procedure for operators to utilize when clearing blockages of equipment lines. Employees were required to clear blockages in equipment lines and sample ports without a written procedure, exposing employees to burn hazards from acid and hot glycol.

The employer shall establish and implement written procedures to maintain the on-going integrity of process equipment. a) Arkema-Sartomer West Chester, PAD A: On or about March 23, 2023, the employer did not ensure written procedures for inspecting and testing storage vessels were implemented to maintain the on-going integrity of process equipment such as, but not limited to, C-155 as required by API 510. The employer did not implement procedures for determining the corrosion rate, fitness for service evaluations based off the thickness determination rates, exposing employees to fire and explosion hazards posed by heptane and toluene in the event of an uncontrolled release.
