29 CFR 1910.333(a)(2): Where exposed live parts were not deenergized (i.e., for reasons of increased or additional hazards or infeasibility), other safety-related work practices were not used to protect employees who could be exposed to the electrical hazards involved: a) Electrical Service Building 1 - On or about March 16, 2023 and at times prior thereto, 13.8 kV bussing that was located behind shutters/guards was not completely guarded against accidental contact. An employee contacted the live parts while performing preventative maintenance resulting in a hospitalization for electric shock/burns.

29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye: a) Facility - On or about March 23, 2023 and at times prior thereto, the employer did not report an incident within 24 hours, resulting in hospitalization, that occurred on March 16, 2023. The incident was referred to the area office by the employer approximately 56 hours after the incident.
