29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift: On or about March 23, 2023, and at times prior thereto, an employee was exposed to a fall hazard while connecting the meter loop onto the exterior of the building at a height over 10 feet without using a personal body harnesses and lanyard to be able to anchor from the aerial lift.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: On or about March 23, 2023, and at times prior thereto, employees were exposed to a fall hazard while performing electrical work from a Vehicle Mounted Aerial lift without having training on fall hazard recognition such as but not limited to fall protection system in place while using the aerial lift.
