Location: 43 Pond Street, Marblehead, MA;  On or about 03/24/23, an employee was allowed to work from a portable extension ladder which was not erected in a manner to provide a 4:1 ratio of the working length of the ladder. The accident scene was recreated by jobsite foreman in the presence of CSHO, company owner, and Marblehead PD, and the top support of the ladder aligned with the scrape marks along the side of the residence.
