29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.  On or about March 21, 2023, portable ladders used to access the roof did not have the side rails extending more than three feet above the roof working surface at the following instances:   a) Two extension ladders on front side of home  b) Extension ladder on the second floor roof on west side of home  c) Two extension ladders to the first story on east side of home

On or about March 21, 2023, portable ladders safety training and a ladder program was not provided to employees who are required to access/egress ladders as part of their work functions.       a) Two extension ladders on front side of home where not extended and employees exposed to fall hazards b) Extension ladder on the second floor roof on west side of home not extended and employees exposed to fall hazards c) Two extension ladders to the first story on east side of home not extended and employees exposed to fall hazards
