1910.212(a)(2)  General requirements for machine guards. Guards shall be affixed to the machine where possible and secured elsewhere if for any reason attachment to the machine is not possible. The guard shall be such that it does not offer an accident hazard in itself.   a) On or about 03/19/2023, on the 242 Die Cutter Line; where the point of operation guard did not adequately prevent employee access to the point of operation on the Martin Quik-Break II Bundle Breaker.  The point of operation guard was constructed and installed in a manner that allowed employees to go under or around the guard to access the point of operation. Employees were exposed to crushing hazards.   ABATEMENT DOCUMENTATION REQUIRED
