29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  On or about 22 March 2023, at US Coast Guard Rio Bayamon Housing MWR building, Guaynabo, PR.   Employees engaged on painting operations have been utilizing chemicals such as primer, paints, gasoline, primercrete and other chemicals  without training on the chemicals they work with.     NOTE:  Certification of abatement is not required for this citation item since it was submitted prior to citation issuance.

29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:   On or about 14 March 2023, at US Coast Guard Rio Bayamon Housing MWR building, Guaynabo, PR.  At a light construction site, employees engaged in water spraying and painting operations were not trained in the recognition on the hazards related to falls, personal protective equipment and chemical hazards and the procedures to control or minimize those hazards.      NOTE:  Certification of abatement is not required for this citation item since it was submitted prior to citation issuance.

29 CFR  1926.95(a):  Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided:  On or about 22 March 2023, at US Coast Guard Rio Bayamon Housing MWR building, Guaynabo, PR.  At the premises of the MWR and Swimming Pool buildings, employees engaged on painting related operations were observed performing water spraying and painting operations without gloves, hard hats, eye glasses and safety shoes.    NOTE: Certification of abatement is not required for this citation item as abatement was documented by the compliance safety and health officer during the inspection.

29 CFR  1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  On or about 14 March 2023, at US Coast Guard Rio Bayamon Housing MWR building, Guaynabo, PR.  Employees engaged on water spraying operations were utilizing a ladder extended just two (2) feet above the upper landing zone to gain access to the roof at a height more than 15 feet from the ground level to continue work.    NOTE:  Certification of abatement is not required for this citation item since it was submitted prior to citation issuance.
