29 CFR  1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.   a. On or about 03/22/2023, at the jobsite located at 11571 Villa Grand, Fort Meyers FL 33900, employees were exposed to fall hazards of approximately 30 to 35 feet, in that,  the employer did not have fall protection certification of training for employees conducting roofing work.

29 CFR  1926.602(d):Powered industrial truck operator training. The employer did not certify that each operator had been trained and evaluated as required by paragraph 29 CFR 1910.178(l):  a. On or about 03/22/2023, at the jobsite located at 11571 Villa Grand, Fort Meyers FL 33900,  the employer exposed employees to crushing hazards in that an employee operated a powered industrial truck without being certified.

29 CFR  1926.1053(b)(5)(i): Non-self-supporting ladders were not used at an angle such that the horizontal distance from the top support to the foot of the ladder way approximately one-quarter of the working length of the ladder (the distance along the ladder between the foot and the top support).   a. On or about 03/22/2023, at the jobsite located at 11571 Villa Grand, Fort Meyers FL 33900, employees were exposed to fall hazards of approximately 30 to 35 feet, in that, the angle of the extension ladder in use to gain access to the roof was too steep, 77.5 degrees.

29 CFR  1926.501(b)(11):"Steep roofs." Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels were not protected from falling by guardrail systems, safety net systems, or personal fall arrest systems.  a. On or about 03/22/2023, at the jobsite located at 11571 Villa Grand, Fort Meyers FL 33900, employees were exposed to fall hazards of approximately 30 to 35 feet, in that, fall protection was not used while engaged in roofing activities while on the 5:12 pitch roof.   National Roofing of Collier Inc. was previously cited for a violation of an equivalent Occupational Safety and Health Standard 29 CFR 1926.501(b)(13) , which was contained in OSHA inspection number 1451125, citation number 1, item number 1 and was affirmed as a final order on 04-17-2020, with respect to a workplace located at 1520 Oakes Blvd., Naples, FL 34119.

On or about 03/22/2023, at the jobsite located at 11571 Villa Grand, Fort Meyers FL 33900, employees were exposed to fall hazards of approximately 30 to 35 feet, in that, the angle of the extension ladder in use to gain access to the roof was too steep and the employer did not ensure that each employee using the  ladder  had been trained by a competent person in the proper placement of ladders.
