29 CFR  1926.102(a)(1):The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:   On or about May 2, 2023, an employee was exposed to struck-by hazards while using a pneumatic nail gun while installing dry wall onto a commercial construction building without eye protection.

29 CFR  1926.451(c)(2)(iv):Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use:  On or about May 2, 2023, and times prior thereto, an employee was exposed to a fall hazard while utilizing an elevated temporary wooden platform supported by a fork lift to install dry wall onto a commercial construction site.
