OSH ACT of 1970 Section (5)(a)(1):The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees a. On or about March 24, 2023 and at times prior, RSV Inc at a work site located at 605 Commercial St., Mingo Junction, OH 43938, the employer did not ensure that personnel were prohibited from being lifted in the bucket of a John Deere Model 250 GLC hydraulic excavator. An employee was exposed to a fall hazard of over 12 feet and caught-in/struck by hazards when being lifted in the bucket of the excavator during demolition activities.
