29 CFR 1910.252(a)(2)(iii)(A)(1): The employer did not provide fire watchers whenever welding or cutting was performed, where appreciable combustible material in building construction or contents was closer than 35 feet (10.7 m) to the point of operation:  On or about April 27, 2023, near Van Line 3 Station 20 of the South Plant, employees were performing welding operations on metal slides where combustible materials were within 35-feet:  a.) Cardboard storage boxes were in proximity of the point of operation of the welder.   b.) Pallets were being stored inside of the welding area.

29 CFR 1910.252(b)(2)(iii): Workers and other persons adjacent to the welding area were not protected from the rays by noncombustible or flameproof screens or shields:  In the South Plant, employees performing welding operations were not adequately protected from welding rays due to flash burns from the lack of appropriate screens in the following areas:  a.) On or about March 21, 2023, on Van Line 3 Station 30, where employees welding the hangers for the slides.   b.) On or about May 1, 2023, on Van Line 1 Station 20, where employees were welding flat and downhill welds.   c.) On or about May 3, 2023, on Van Line 4 Station 10, where employees were welding flat and downhill welds.  d.) On or about May 9, 2023, on Van Line 3 Station 50, where employees were welding flat and downhill welds.   e.) On or about May 15, 2023, on Van Line 4 Weld Out Station, where employees were placing bolts into the steel slides.  The SAF-Holland, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.252(b)(2)(iii), which was contained in OSHA inspection number 1340459, citation number 1, item number 7 and was affirmed as a final order on February 28, 2019, with respect to a workplace located at 1103 S. Main St, Dumas, AR 71639.

29 CFR  1910.252(b)(3):  Employees exposed to the hazards created by welding, cutting, or brazing operations were not protected by personal protective equipment in accordance with the requirements of 29 CFR 1910.132:  a.)  On or about March 21, 2023, at Station 20 on Line 3, each employee performing MIG welding was not provided with protective equipment such as but not limited to fire retarding clothing when to cover the body and extremities.  b.)  On or about April 27, 2023, near Van Line 3 Station 20 of the South Plant, Flame-resistant (FR) jackets were not maintained in a sanitary and reliable condition in that they were being stored on top of tools inside of the welding area.  The SAF-Holland, Inc. was previously cited for a violation of this occupational safety and health standard 1910.252(b)(3), which was contained in OSHA inspection number 1340459, citation number 2, item number 2 and was affirmed as a final order on February 28, 2019, with respect to a workplace located at 1103 S. Main St, Dumas, AR 71639.
