29 CFR  1910.212(a)(2): Guards were not affixed to the machine where possible or secured elsewhere when attachment to the machine was not possible. Machine guards on machines were so installed that they offered accident hazards in themselves:   a. Food Bank of the Rockies, Inc.: On March 13, 2023, the employer did not protect employee from hazards created by ingoing nip points. Employees operated the Oliver Lidder Model 1908 without the use of machine guards. The presence sensing control device present was not operating and an employee sustained a crushing injuries to their hands. This condition exposed employees to crushing and amputation hazards.
