29 CFR 1926.651(b)(3): When excavation operations approach the estimated location of underground installations, the exact location of the installations shall be determined by safe and acceptable means. On or about March 27, 2023, the employer did not determine the exact location of utilities by a safe and acceptable means and made contact with a gas line with an excavator. This condition exposed employees to a fire hazard.
