29 CFR 1926.451(c)(2):Supported scaffold poles, legs, posts, frames, or uprights did not bear on base plates and mud sills or other adequate firm foundation: Location: 12 Pope Street, Salem, MA On or about 3/28/23, during the removal of a mast climbing scaffold, the employer did not ensure that the supported scaffold was on adequate firm foundation. A base plate with attached sections of mast, along with the motorized climbing unit was placed on the ground during removal, with the motorized climbing unit in an elevated position. The unit subsequently tipped over, pinning an employee.

29 CFR 1926.602(c)(1)(ii):Modifications or additions which affect the capacity or safe operation of the equipment were made without the manufacturer's written approval: Location: 12 Pope Street, Salem, MA On or about 3/28/23, the employer did not ensure that a lifting attachment that would suspend a load below the forks of a rough terrain powered industrial truck was approved for use. An attachment that slid over one fork contained a hook that allowed equipment to be suspended by a sling, which would result in an un-centered load.
