29 CFR 1926.502(b)(2)(i):Midrails, when used, were not installed at a height midway between the top edge of the guardrail system and the walking/working level; screens and mesh, when used, did not extend from the top rail to the walking/working level and along the entire opening between top rail supports; Intermediate members, when used gbetween posts, were more than 19 inches (.5 m) apart; Other structural members were installed such that there are no openings in the guardrail system that are more than 19 inches (48 cm) wide. 15506 Interplace Road, Bldg#3, San Antonio, TX.: On or about March 27, 2023 and at times prior thereto, workers performing framing and truss installation were exposed to fall hazards from a perimeter guardrail system that had midrails that were not installed at a midway height between the top rail and the walking surface.
