Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. a) Jobsite located at 2004 Sproul Road, Broomall, PA. - The employer did not ensure that employees conducting roofing operations from the roof of a commercial building, approximately 30 feet above the ground level, with a pitch of 3:12 were protected from fall hazards by the use of a fall protection system. Occurred on or about March 28, 2023
