29 CFR 1910.28(b)(1)(i):(1) Unprotected sides and edges. (i) Except as provided elsewhere in this section, the employer must ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the following: (a) On or about March 28, 2023, located above the shop bathroom, employees conducting filter exchange activities were not protected from falling to the lower lever. This exposed employees to a fall hazards of approximately ten feet.

29 CFR 1910.254(d)(9)(iii):Cables with damaged insulation or exposed bare conductors shall be replaced. Joining lengths of work and electrode cables shall be done by the use of connecting means specifically intended for the purpose. The connecting means shall have insulation adequate for the service conditions. (a) On or about March 28, 2023, the employer did not ensure the damaged insulation and exposed bare conductors of a Miller, Millermatic WF-64 MIG welder machine's flexible electrical grounding cable were replaced. The grounding cable did not have insulation adequate for the service conditions, exposing employees to electrical hazards.

Except as provided in paragraphs (f)(7) and (f)(8) of this section, the employer shall ensure that each container of hazardous chemicals in the workplace is labeled, tagged or marked with either. (a) On or about March 28, 2023 the employer did not ensure all secondary containers of hazardous chemicals were labeled within the facility, such as but not limited to Spray Clean SS99 chemical and Tough Gard Anti-Spatter liquid.
