29 CFR  1910.212(a)(3)(ii): The employer did not ensure the point of operation of machines whose operation exposes an employee to injury was guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle.   a) At the facility, 1120 Hook Road, Farmington, NY - Cutoff Department. On or about March 28, 2023, employees were operating the Sever All saw without the point of operation guarded. The employer did not ensure the point of operation was guarded.    b) At the facility, 1120 Hook Road, Farmington, NY - Cutoff Department. On or about March 28, 2023, employees were operating the Stone saw without the point of operation guarded. The employer did not ensure the point of operation was guarded.   c) At the facility, 1120 Hook Road, Farmington, NY - Mill Area. On or about March 28, 2023, employees operated Bridgeport Mill No. 28 without guarding to protect against the point of operation and rotating bit. The employer did not ensure the point of operation of the machine whose operation exposes an employee to injury was guarded.  d) At the facility, 1120 Hook Road, Farmington, NY - Sheet Metal Area. On or about March 27, 2023, an employee operated the Amada Press No. RG100S without point of operation guarding. The employer did not ensure guarding at point of operation.  e) At the facility, 1120 Hook Road, Farmington, NY - Sheet Metal Area. On or about March 28, 2023, an employee operated the Amada Press, No. 8020E without point of operation guarding. The employer did not ensure guarding at point of operation.  f) At the facility, 1120 Hood Road, Farmington, NY - Sheet Metal Area. On or about March 27, 2023, an employee operated Promecan Press, No. R5-75-12 without point of operation guarding.  The employer did not ensure guarding at point of operation.  Ingleside Machine Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 1370981, citation number 1, item number 2 and was affirmed as a final order on 3/11/2019, with respect to a workplace located at 1120 Hook Road, Farmington, NY 14425.
